Laparoscopic surgery for gallstones/chronic cholecystitis/biliary dyskinesia and acute inflammation of the gallbladder due to gallstone disease in past twenty years became a standard of care with very few open procedures performed. It is a gallbladder surgery that demonstrated and confirmed all the advantages of minimally-invasive surgery to surgical community. Aklmost evry general surgeon is currently trained to perform this surgery.
At the same time not every surgeon id adequately trained to deal with this surgery in morbidly obese patients, patients who had bariatric surgery in the past or prepared to deal with extremely difficult situation when acute inflammation progressed to the point that laparoscopic surgery is impossible and dangerous and conversion to open surgery is warranted.
Laparoscopic gallbladder surgery is one of the most frequent surgeries we perform mostly in difficult to treat patients. Despite that there were no mortality or severe complications in several hundreds of patients we treated.
In patients undergoing bariatric surgery rapid weight loss is associated with increased frequency of gallstones and gallbladder dysfunction (or dyskinesia). This can happend after any bariatric procedure and general surgeons often are not prepared to deal with gallbladder problems in bariatric patients. After gastric bypass if gallstones escape into the main duct connectiong liver and intestine (common bile duct) vast majority of surgeons will resort to open procedure with all attendant risks.
